• Agent Contracting
  Broker/Producer who doesn’t work under an agency and/or commissions are paid directly to the Broker/Producer.
• Agency Sub-Agent Contracting
  Broker/Producer who works under an agency and commissions are paid to the agency.
• Agency and Principal Contracting
  Principal/Authorized Officer for Agency when request is to set up a new agency. Log in using the agency principal SSN and name. Then indicate the agency name and Tax ID in Section 3 of the Producer Appointment Data Sheet.

  You will also need to attach a copy of your license and current E&O.
